WHAT IS THE FRAXEL- PDT (PHOTODYNAMIC THERAPY) COMBINATION TREATMENT AND WHO CAN IT BENEFIT?

One of the most innovative treatments that ADAS providers are helping to pioneer is a procedure called the Fraxel™-PDT Combination treatment. This technique uses Fraxel™ laser to gently permeate (break) the skin barrier followed by the application of an odorless chemical- ALA, short for aminolevulinic acid. ALA is a great topical drug which can help to identify skin precancers before they become significant skin lesions or even squamous cell carcinoma. This drug is also preferentially absorbed by both oil glands and acne causing bacteria, so the effects on excess oil production and acne conditions are simply put...wonderful. To summarize this novel procedure is capable of treating:

HOW IS THE TREATMENT ACTUALLY PERFORMED?

The procedure begins by an ADAS provider performing a two pass Fraxel™ treatment. This is carried out to allow for the ALA to penetrate evenly. Next, the ALA is applied to the skin and allowed to incubate for 1 hour. This is a painless and odorless application.

Following this, we take either a Blue light or second laser, a pulse dye laser to activate the chemical. This laser induces an even redness and causes a minimal disruption of the top layer of the skin. Revamping occurs quickly and the skin takes on a fresher and more invigorated appearance within a few days. Two to five treatments are needed to see the desired cosmetic effect.

One must remember to stay out of the sun or intense indoor lighting for up to 72 hours as the ALA can remain active during this period. Great results require some patience and sometimes a few treatments... but this combination may be just be what the doctor ordered for those with moderate sun damaged skin and/or acne.

HOW LONG DOES THE ACTUAL TREATMENT TAKE? HOW MANY SESSIONS ARE NEEDED?

The Fraxel-PDT Combination treatment takes about 90 minutes as ALA is left on the skin for about 60 minutes to allow for adequate absorption. Usually 2 to 7 treatments sessions are required to see a 50 to 95% improvement. These are spaced out every 4 weeks (on average).

WHAT IS THE PRE CARE INSTRUCTIONS FOR THE FRAXEL-PDT (PHOTODYNAMIC THERAPY) TREATMENT?

Typically, we ask our patients to discontinue the use of retinoic acid products and glycolic acid products for 3 to 4 days before your next treatment. It is also wise to stay out of intense sunlight (or tanning beds) beginning 1 week prior to your laser treatment. Sometimes an antiviral medication may be prescribed if you are prone to cold sore formation.

HOW MUCH DISCOMFORT CAN I EXPECT FROM THE FRAXEL-PDT (PHOTODYNAMIC THERAPY) TREATMENT?

There is mild to moderate discomfort with the Fraxel-PDT (Photodynamic Therapy) Treatment. A topical or local anesthetic is all that is usually required. Topical anesthetics are applied for 30 minutes and covered with Saran Wrap™ to enhance the cream's effectiveness. Pain usually is described as a modest burning, or stinging. Cool compresses and medications such as Motrin™ or Advil™ can readily help to alleviate any discomfort.

HOW LONG DOES THE SKIN TAKE TO HEAL?

Fraxel-PDT Combination Treatment is mildly traumatic to the skin. Bruising and mild to moderate swelling is often noted post op. Depending on the location and treatment type, these processes can take up to 3 to 7 days to heal. Once the skin has fully healed, you can then undergo another treatment.

WHAT ABOUT POST OP CARE?

We recommend a post laser skin care system. We also sometimes prescribe a corticosteroid pack to help reduce the duration of swelling. As mentioned earlier, remember to stay out of intense sunlight as you will remain photosensitive for up to 72 hours after the procedure. We usually recommend that you are out of the sun for the week after your Fraxel-PDT treatment has been performed. Concealing makeup, e.g. Jane Iredale™, can be used to minimize redness or bruising.

HOW LONG WILL THE BENEFITS FROM FRAXEL-PDT (PHOTODYNAMIC THERAPY) TREATMENT LAST?

The benefits from the Fraxel-PDT procedure can last for a long time. Repeat treatments are sometimes required to further enhance your results but this is determined on a case by case basis. In our experience, patient satisfaction has been extremely high for this procedure. Results, however, may vary from patient to patient.
